The National Transportation Safety Board determines that the probable cause of the collapse of the emissions control barge STAX 1 capture and control articulated arm was a shoreside crane operator moving a ship-to-shore container crane without verifying the crane’s lowered boom had ample clearance over any obstructions, and the container terminal’s inadequate guidance for gantrying cranes.
