A船は、船長A及び航海士Aほか8人が乗り組み、メチルエチルケトン約920tを積載し、自動操舵で遠州灘を西進中、航海士Aと共に船橋当直に就いていた甲板手Aが、平成24年3月8日04時30分ごろ左舷船首約10°2~3M付近にB船の灯火を視認し、航海士Aに報告した。航海士Aは、ゆっくり西進しているように見えたB船が、その後、進路を右に変えたように見えたので危険はないと思い、エクササイズをして目を離していたところ、甲板手Aからの報告により、右舷船首方の至近距離にB船を認め、直ちに機関を減速して手動操舵に切り換え、衝突を回避しようとしたが、04時49分ごろ、渥美半島南東方沖において、A船の船首部とB船の右舷船尾部とが衝突した。船長Aは、機関の回転数が落ちたことに気付いて昇橋し、航海士AからA船がB船と衝突したことを聞いて周囲の状況を確認後、ジェネラルアラームにより、全乗組員を招集した。 A船は、
