A船は、船長A及び二等航海士Aほか19人が乗り組み、石炭20,000tを積載し、平成25年1月24日未明筑前大島北西方沖を関門港に向けて東進していた。 船長Aは、二等航海士Aが船橋当直中の02時15分ごろ、関門海峡海上交通センターへの通報準備等のために昇橋し、A船の周囲を確認した後、明かりを点けた海図台の区画のカーテンの中で事務作業を行っていた。 二等航海士Aは、甲板員Aと共に見張りを行いながら、自動操舵で東進していたところ、02時28分ごろレーダーでA船の左舷船首方から接近するB船を探知し、ARPAでこの映像を捕捉した。 二等航海士Aは、B船がA船の後方を通過すると判断したが、最接近距離が近いので、携帯式昼間信号灯を使用してB船に照射を繰り返していたところ、B船が避航する様子を見せないので、甲板員Aに右舵を取らせた。 船長Aは、二等航海士Aの操舵号令を聞いて海図台の区画から出たところ、至
