本船は、船長ほか18人が乗り組み、石炭約18,200tを積載し、船首約8.36m、船尾約8.54mの喫水で、平成27年1月23日12時30分ごろ山口県下関市六連島北方沖で水先人が乗船し、関門航路に向けて航行した。水先人は、操舵室において船長が操船指揮、航海士が機関操縦及び甲板手が手動操舵の配置で水先業務中、タグボート(約2,500kW)を右舷船首及び別のタグボート(約2,600kW)を右舷船尾に取り、13時31分ごろ関門航路から関門港小倉区の西1号岸壁(以下「本件岸壁」という。)に向けて右転し、機関を極微速力前進とし、約5ノット(kn)の速力(対地速力、以下同じ。)で南進した。水先人は、本件岸壁手前で右転して本件岸壁に左舷着けとするように右舵を取ったが、右転しなかったので機関を微速力前進とした。水先人は、微速力前進としても右転せず船首が岸壁角に向いていたので、機関を止め、すぐに後進に
