Dorothy Ann / Pathfinder, Cleveland, OH, USA (NTSB DCA25FM013)Report published Feb 3, 2026

The National Transportation Safety Board determines that the probable cause of the diesel generator engine failure and resulting fire aboard the self-unloading barge Pathfinder was a loss of lube oil to one of the engine’s connecting rods, possibly due to its electrical generator experiencing a sudden block load beyond its design capability resulting from an electrical short in the generator.

The report names a ship "Dorothy Ann" but prints no IMO number; the name matches this vessel only. It may be a different ship.

Behaviour at sea Voydar-derived

Patterns Voydar derived from this ship's AIS broadcasts. They are inferences, not reports: each one shows how it was computed and how confident we are, and none of them says why a ship behaved this way.

Draught change

Reported draught went from 6.7 m to 5.9 m after leaving Cleveland. Voydar infers the ship probably discharged cargo.
