A船は、水島港のJFEスチール西日本製鉄所(以下「JFE」という。)南側海域を速力(対地速力、以下同じ。)約12ノット(kn)で西北西進中、平成24年9月25日16時23分ごろ徐々に減速し、16時31分ごろ、JFE西側の掘下げ水路に入って間もなく、A船の右舷船尾部とB船の船首先端部とが衝突した。 B船は、船長Bほか3人が乗り組み、熱延コイル約687tを積み、阪神港に向け、16時10分ごろJFE岸壁を離れ、船長Bが、機関を半速力前進にかけて徐々に増速しながら、JFE西側の掘下げ水路を手動操舵で南進した。 船長Bは、16時27分ごろ約10.9knの速力で航行中、左舷前方に前路を横切る態勢で接近するA船を認め、A船の船尾方を通過しようと思い、直ちに機関を中立とし、その後、機関を後進にかけた。 B船は、減速しながら、針路を保持して航行中、A船と衝突した。
